- Your name
- Order number
- SKU number of the item that is missing
- Quantity of the item you are missing
- Photo of your packing slip
- If you have received an incorrect item, please also include a photo of the item you received in it's place.
- If you are missing an item from a LCD kit, please include a photo of the items you DID receive.
