  • SKU number of the item that is missing
  • Quantity of the item you are missing
  • Photo of your packing slip
  • If you have received an incorrect item, please also include a photo of the item you received in it's place.
  • If you are missing an item from a LCD kit, please include a photo of the items you DID receive.
